MIR rebate forms available

I have extras of the following forms available. I'm asking for 1 forever stamp for each form PLUS a SASE (self-addressed, stamped envelope). If both of those criteria are not met, I won't fulfill my end of the bargain. I don't want to be a hard-nose but I've gotten burned many times and it's ended up costing me to mail out forms. I'm happy to share my extras but not at my own expense.
